Referral Process for Children Ages 3 to 21 years

Should you feel your child may possibly need special education services you have the right to refer them for a special education evaluation through  your school district. The referral process is listed below. If at any time you have any questions, please call Educational Service Unit 5, Special Education Department at 402-223-5277.

1) Contact your school district or Educational Service Unit 5 at 402-223-5277. You will be sent a Consent for Evaluation form to sign and return to your school or ESU 5.

2) The district will complete the referral form, the rest of the referral paperwork and evaluator(s) will be contacted to do the evaluation of your child. The evaluation will be completed.

3) Parent will be contacted to attend a Multi-Disciplinary Team meeting (MDT) to determine whether or not your child has a verification. The evaluation and MDT meeting must be held within 45 school days of the date that parent signed Consent for Evaluation form.

4) If your child has a verification, an Individualized Educational Plan (IEP) meeting will be scheduled within 30 school days after the MDT meeting to discuss implementation of services your child may need. (It may possibly be scheduled along with the MDT meeting.)  If it is determined that your child does not qualify for services, they will be referred to the School Assistance Team (SAT) to develop accommodations, strategies and/or a 504 Plan to assist your child with his/her learning needs.
